Updated:  This issue is resolved.  If you are continuing to have issues, please contact the Help Desk at x7440.   LITS has identified an issue with a server that enables authentication (logins) to some third party services including Hathi Trust, Lynda.com, and Interfolio.  Email/Office 365 is NOT affected. Forms in Office 365

Forms is one of the new apps available in Office 365 after August 15. Forms makes it possible to create online forms, similar to those created in tools like Google Forms, SurveyMonkey, or Wufoo.
